L’Anse Creuse North Athletics has implemented a new platform called FinalForms to make student-athlete registration and the collection of required forms and medical information easier and more organized. Parents/guardians will use FinalForms to register their student and submit all required information, including a current sports physical. Please follow the attached directions for step-by-step instructions on how to upload your student’s physical to FinalForms. **Important Physical Requirements:** - Physical examinations must be completed on or after **April 15 of the current school year** to be considered valid. - Students must have a valid physical on file before trying out for their sport. **No physical = No tryout or participation!** If you have questions or need assistance, please contact LCN Athletics.
